Going Home Faster when Self-Monitoring is Part of the Plan

Hospital stays are expensive and patients would usually rather be at home, where they can get a good night of sleep and eat home-cooked meals. With new technologies (and the fact that aging Boomers are quick to adopt them), recovering at home is a real possibility.

Self-monitoring at home and sharing the results electronically is a growing trend. Patients are trained to take their vitals. The data is then transmitted electronically or over the phone. Nurses check the data and enter it into the patient’s digital personal health record (PHR). That PHR is available to anyone on the patient’s care team, so everyone has current and accurate information about the patient.

While hospitals will always be needed, it’s likely that as new approaches to caring for chronic conditions catch on, they will focus more on acute care and less on general care.
